REQUEST FOR STATEMENT OF QUALIFICATIONS FOR THE TOWN OF PLAINVILLE, INDIANA
In order to assure compliance with the Indiana Office of Community and Rural Affairs and related requirements regarding competitive negotiation procurement of professional services, specifically engineering and inspection services, the Town of Plainville is requesting “Statements of Qualifications” for the provision of design and technical assistance ser-vices.
Description of Pro-ject:
The project consists of a Stormwater Drainage construction project for the Town. This includes design and construction of utilities for development of the recommended activities provided from previous studies and analysis by a consultant of existing conditions. The proposed project development and construction will potentially be funded by Disaster Recovery Appropriation funding or from Commun-ity Focus Funds from the Indiana Office of Com-munity and Rural Affairs.
Description of Ser-vices Needed:
Professional engine-ering services are needed to undertake and perform technical assistance necessary to complete the proposed project. These services include, but are not limited to, preparation of site evaluation guidelines, preparation of preliminary engineering and bidding specifications for the construction, mapping, and overall general administration and inspection representation of project.

Type of Contract:
The Town of Plainville will execute a lump-sum contract for this project. All services will be stated on a cost reimbursement basis (hourly rate and maximum hours fixed). The awarded contract will have a “not-to-exceed” clause which will limit the allowable fees charged in connection with this project through close out.
Federal Require-ments:
The project will be developed in a manner which satisfies all applicable Federal, State, and Local regulations. Pro-spective organizations should note the successful engineering firm must meet the terms and conditions outlined in the “Federal and State Third-Party Contract Provisions”, which are attached to and made part of this RFQ.
The Indiana Office of Community and Rural Affairs has a 10% MBE/DBE/WBE goal for all projects funded with Community Development Block Grant funds.
Statement of Qualifi-cations:
Each prospective organization must submit a Statement of Qualifications which includes the following:
1. Name, address and brief description of firm.
2. Resumes of key personnel to be assigned in this project as well as a list of professional staff members.
3. A description of expertise, experience, and resources directly relevant and available for the proposed project.
4. A list of similar projects previously completed.
5. A list of references.
6. Description of scope of services.
7. Description of firm’s proposed approach to the study and how it intends to provide the full range of engineering services necessary to assure the correct, complete and timely execution of the project to the satisfaction of the City and in full compliance with federal and state laws, rules and regulations.
The Statement of Qualifications shall also provide the following information: name, title, address and telephone number of individuals with authority to negotiate and bind the applicant contractually, and who may also be contacted during the period of proposed evaluation.
Evaluation Criteria:
Statement of Qualifi-cations evaluation criteria shall include:
1. Specialized exper-ience or technical expertise of the organization and its personnel in connection with the scope of services to be provided and the complexity of the project. (50 points)
2. Past record of performance on contracts with the community and other clients, including quality of work, timeliness and cost control. (50 points)
3. Capacity of the organization to perform the work within time limitations, taking into consideration the current and planned workload of the firm. (50 points)
4. Familiarity of the organization with the project and the type of problems applicable to the project. (50 points)
Selection of profes-sional consultants by the Town of Plainville is based on competitive negotiation, with evaluation based on the above criteria. An evaluation sheet is attached which illustrates the method to be used for scoring each Statement of Qualifications and the number of points possible for each criteria. Interviews will be conducted with one or more offerors for the purpose of clarification, assuring complete understanding of and responsiveness to the solicitation requirements. Award will be made by the Town to the offerors whose qualifications will be most advantageous to the Town. All firms who submit a Statement of Qualifications will be notified after an award is made.
If you are interested in providing the required services, please note the Statement of Qualifications (1 original with 5 copies) of each prospective organization must be received at the Town Council President, Lee Coffman, P.O. Box 177, Plainville, IN 47568 no later than 4:00 p.m. on March 11, 2010. The Statement of Qualifications must be received in a sealed envelope marked “STATEMENT OF QUALIFICATIONS FOR PLAINVILLE STORMWATER DRAIN-AGE.” Statements of Qualifications received after the deadline will be returned unopened. Each Statement of Qualifications will be reviewed for completeness and clarity according to the criteria listed herein.
The Town may or may not negotiate the scope of services with one or more offeror(s). The Town reserves the right to reject any or all solicitations. The Town is an Equal Oppor-tunity Employer. The Town anticipates negotiation of a contract by the middle of April. A contract will not be formally awarded and executed until given such notice and authorization from the OCRA to do so.
