PUBLIC PUBLIC Destruction of Winona Memorial Hospital a/k/a WMH Physicians Hospital, LLC, Patient Medical Records
Winona Hospital (a/k/a WMH Physicians Hospital, LLC) declared bankruptcy and ceased business operations on or about September 8, 2004. The bankruptcy trustee retain-ed the patient medical records for a perior of one year. Currently, the Indiana Residual Malpractice Insu-rance Authority (“IRMIA”) maintains Winona Hos-pital’s patient medical records. IRMIA intends to destroy all Winona Hospital patient records and x-ray films starting January 1, 2010.
Former patients who want their records must request their medical records by 5:00 p.m., Tuesday, December 1, 2009, and must also pick them up from Iron Mountain in Indianapolis, Indiana, by 3:00 p.m., Tuesday, December 15, 2009. Copy and retrieval fees may apply.
Arrangements for ob-taining medical records may be made by calling Iron Mountain at (317) 554-1063 during normal business hours. Two forms of personal information must be verified before records will be released to anyone. Please be advised that patient records will not be disbursed to family members except when appropriate authorization is provided.
